Insurance that requires referrals and authorization:

1. Aetna EPO needs referral from Primary Care Provider (PCP)
2. Kaiser needs referral on file and authorization
3. Tricare needs referral and authorization obtained from Obstetrician

All copays, co-insurance and deductibles are collected at the time of the visit as per the contract with your insurance company.
